The Health Care Program for Children with Special Needs (HCP) is a unique resource for families, health care providers and communities. HCP provides information, referrals, support, and care coordination services to children with special health care needs. HCP serves children from birth to 21 years of age that have, or are at risk for, serious physical, behavioral or emotional conditions.
The HCP program works with families, providers, and the community to connect kids with the care they need. There are no financial or diagnostic criteria, any child with a special health care need may receive services.
